Employer Provided Group Life Insurance-
Eligibility – All full time employees after 2 years of service
Coverage- One and half times annual salary, plus accidental death and dismemberment
Employer Provided Group Disability Insurance-
Eligibility- All full time employees after 2 full years of service
Coverage- 60% of first $50,000 of annual salary, plus 40% of any salary in excess of $50,000
Employer Retirement Plan- Defined Contribution Plan-
Eligibility- Any employee age 21 and older that has completed 2 years of service.
Employer Contribution- 8% of gross monthly compensation
